Dual health plans, or Dual Special Needs plans, are for people who qualify for both Medicare and Medicaid. Dual health plans (D-SNP's) include extra benefits to help ease the challenges faced by dual-eligible members. Some of these extra benefits cover care and services that go above and beyond Medicaid and Original Medicare.* Other services make it easier for dual health plan members to manage their care. Wouldn’t it be nice to have someone to help guide you and help answer your questions? A Navigator is your go-to person to help you every step of the way on your health journey. There’s no cost for this service.

Just like the name says, your Navigator can help you:

  • Make a plan to help with questions about your medical, transportation and food needs
  • Schedule appointments and transportation to your doctor, dentist or pharmacy
  • Review your benefits to help you get the most out of your plan
  • Help make sure you and the people who take care of you at home have what you need

A Navigator is your go-to person for support

For dual health plan members, a Navigator serves as your single point of contact. Your Navigator is a trusted advisor. Someone who’ll learn about your specific health challenges. Your Navigator will work with you, your family and your caregivers to make sure your care needs are met.

It can be a complex, often confusing world for those who are dual-eligible and qualify for both Medicaid and Medicare. Having a dual health plan can help simplify things. But it’s even better to have a Navigator who you can call to get answers and extra support when you need it.
