It might be right if...
- You have diabetes, chronic heart failure, and/or a cardiovascular disorder
- You would benefit from extra care management programs to support your health

It might not be right if...
- You are eligible for a Dual Special Needs plan (D-SNP) in your area
- You do not have a qualifying chronic condition (diabetes, chronic heart failure, and/or cardiovascular disorders)
- You live outside an eligible plan coverage area
Types of Special Needs plans
Chronic Special Needs plans (C-SNPs)
For people living with qualifying chronic conditions
Dual Special Needs plans (D-SNPs)
For people who have both Medicare and Medicaid (called “dual eligible”)
Institutional Special Needs plans (I-SNPs)
For people who live in a skilled nursing facility
A person who lives in a contracted assisted living facility and needs the same kind of care as those who live in a skilled nursing facility may qualify for an Institutional-Equivalent Special Needs plan (IE-SNP). This is a special type of I-SNP.

The healthy food benefit is a special supplemental benefit only available to chronically ill enrollees with a qualifying condition, such as diabetes, chronic heart failure and/or cardiovascular disorders, and who also meet all applicable plan coverage criteria. 2 Optum® Home Delivery Pharmacy and Optum Rx are affiliates of UnitedHealthcare Insurance Company. You are not required to use Optum Home Delivery Pharmacy for medications you take regularly. There may be other pharmacies in our network.
3 You will pay a maximum of $25 for each 1 month supply of Part D covered insulin drug through all drug payment stages except catastrophic drug payment stage where you'll pay $0.
